Freezing of Gait in Parkinson’s Disease

Freezing of gait (FoG) in Parkinson’s disease is a common and disabling motor symptom characterized by a temporary inability to move the feet forward despite the intention to walk. Key features include:

  1. Sudden immobility: Patients may feel as if their feet are “glued” to the ground, especially during walking or when initiating movement.
  2. Triggering situations:
    • Turning: Freezing often occurs when attempting to change direction or turn.
    • Narrow spaces: Walking through doorways, hallways, or other confined areas can trigger freezing.
    • Crowded environments: FoG may be more frequent in busy places with many obstacles or people.
    • Emotional stress: Anxiety or rushing can also trigger or worsen freezing episodes.
  3. Short duration: Freezing episodes usually last a few seconds but can be longer in some cases.
  4. Variability: FoG is unpredictable, and not all patients experience it the same way. It can occur more frequently as the disease progresses.
  5. Postural risks: Freezing often leads to falls due to the inability to adjust posture quickly when balance is compromised.

Management of freezing of gait includes physical therapy, cueing strategies (e.g., stepping over visual lines or rhythmic auditory cues like a metronome), and medication adjustments. In advanced cases, deep brain stimulation (DBS) may be considered for treatment.
