Speech and Voice Changes in Parkinson’s Disease

Speech and voice changes, collectively known as hypokinetic dysarthria, are common in Parkinson’s disease and can significantly affect communication. These changes include:

  1. Soft or weak voice (hypophonia): The voice becomes quieter, making it harder for others to hear the person speaking.
  2. Monotone speech: A lack of pitch variation, resulting in flat or monotone speech.
  3. Reduced speech clarity: Slurred or mumbled speech, making it difficult to articulate words clearly.
  4. Hoarseness: The voice may sound hoarse, breathy, or strained.
  5. Speaking rate: Speech may be slower with pauses between words, or in some cases, it may become rapid and difficult to follow (festinating speech).
  6. Difficulty starting speech: Initiating speech can become challenging, similar to the motor symptom of bradykinesia (slowness in starting movement).
  7. Impaired facial expression: Reduced facial movement (hypomimia) can make the person seem less expressive, affecting both verbal and non-verbal communication.

These speech and voice issues can worsen as Parkinson’s disease progresses, but treatments like speech therapy (e.g., the Lee Silverman Voice Treatment) can help patients improve voice volume and clarity. Assistive devices or communication aids may also be beneficial in advanced stages.
